Default '04 Dakota front bearing issue

Hi all. My truck developed a loud, low-tone whine on the front left side. The mechanic said it was the front-left bearing. I went on YouTube and also looked in the Haines manual and it looked simple enough to fix myself. I purchased a new hub and installed it exactly as described in the manual. The noise went away, so I figured I'd fixed it. Then three weeks later, the noise started returning and now it's almost as bad as before. I've questioned a few people about greasing the hub prior to installation, but they've all said they thought it would be pre-greased. I also called the store where I bought it (Lordco) and they didn't know.

So I have some questions:

1. Should I have packed the bearing with grease prior to installation? (There was nothing in the manual about that; it just said to grease the end of the axle.).

2. Would removing the hub and packing the bearing with grease fix the problem, or is it shot now that it's whining?

3. Could this be a different problem, perhaps the front axle?

All opinions gratefully received and thanks in advance.
